Child Protection System Failures have been thrust into the spotlight as a grieving mother challenges recent inquest findings following the tragic death of her daughter Semina Halliwell. The case has exposed critical concerns about how sexual assault reports from young people are handled by authorities.

Inquest Conclusion Sparks Controversy

Assistant Coroner Johanna Thompson, presiding over Sefton, Knowsley and St Helens, delivered findings that have been met with significant opposition from the bereaved family. The inquest concluded that Semina Halliwell had a complex social history, with her death occurring as a consequence of taking her mother’s prescription medication while in distress. The coroner stated that Semina’s intent at the time remained unknown.

 

Notably, the coroner found “no suggestion of any systemic breach on the part of Merseyside Police” and added that there was no evidence authorities could have prevented Semina’s death by overdose. These conclusions  have been strongly contested by Semina’s family.

Mother’s Fierce Challenge to Findings

Rachel Halliwell, Semina’s mother, has publicly rejected the inquest’s conclusions, highlighting what she perceives as significant failures in the system’s response to her daughter’s case. “My daughter reported a sexual assault. In doing so, she seemed to fall off the radar and through the cracks of policy,” she stated with evident distress.

The bereaved mother’s concerns extend beyond the immediate findings. Speaking to journalists after the inquest, Ms. Halliwell raised serious questions about the scope of evidence considered during the proceedings. “A lot of important evidence had not been allowed at the inquest,” she revealed, adding with determination, “My fight for my daughter continues. Semina lost her life and her voice, and I am here, and I will not stop until I am heard.”
