  • Visit a Post Office equipped with a Passport Check and Send service.
  • Call the Passport Adviceline at 0300 222 0000.

When filling out the form, complete sections 1, 2, 3, 4, and 9. If your child is aged 12 or older, they must sign section 6. Additionally, if your child is 11 or under or their old passport photo is no longer recognizable, you will require a ‘countersignatory’ to fill in section 10 and certify your child’s new photo.
